Well a pic would be nice but try this....
Motor should have 4 leads...
#1 will be a Neutral winding(possibly)white....
#2 Green(ground) same as the AC cord....(might be a screw on the frame)
You'll need to connect these first....

Now Switch: Black wire coming from cord goes to Center Pole of the ON/OFF/ON (this is the Feed from AC outlet)...
The 2 remaining leads from the motor will go to other Poles of the switch(ON Positions)....
Your sure this is a Split Pole AC 120 VOLT Motor? (forward/rev)..?

Place switch in ON Position....
Plug Cord into AC outlet...
....Does motor Run?...
Remove cord from outlet....Place switch in other direction....Place cord in wall...Does Motor run opposite Direction?....
DO NOT HANDLE THE SWITCH OR MOTOR WHEN PLUGGED TO OUTLET!!!
IT IS HOT ALWAYS!....IT CAN KILL YOU.....
If the motor works as required you will need to place the switch in a proper enclosure to safe guard yourself from electric shock...
Then Mount the motor...so you DO NOT Handle it during Operation...
And you will need a in-line fuse block between the AC Cord and the switch...This is to Protect You and the motor....
The Name Plate on the motor should have...FLA(full load amps)...
The fuse shall not be greater than 125% of this value....
EX. FLA /5amps...Fuse Size 6 amps....
Please be care...;)
All the Best

You should be able to use a dpdt switch, take the lead from the motor and connect to the center pole and a jumper from there across to the other center pole and connect your control wires to opposite ends of the switch.

Hopefully the picture will post... Drew it out quickly on my youngest's homework, lol. Since rotation is reversed with the connections of red and black to yellow and terminal 2, I think this is how I would wire up a DPDT switch. Not sure you have room for it in that box, you may need to mount another.

I want to thank all of you for your help with this in case it slips my mind and after 50 years on this earth that can happen :D God Bless.
