ARF 2001 D104-C microphone question

I have a D104-C head and I want to build the circuit that was in the original microphone. I have pics of the board and can identify all but 4 parts. I screen captured these shots and labeled but unfortunately the angles just don’t let ya make out everything.
You will see in the pics the components I can’t figure out the ratings and values of

I just want to see how it turns out.
I will let you know I have the pics of the board and I am going to have some made up so it looks original. Once finished I will post results and pics.
