With nomad's help I found the issue a smd resistor covered by a bad solder joint on IC5 put in at the factory by mistake and caused the issue.

Oooooh! That was well spotted!! I would have NEVER found that kind of issue...
So you removed the 22K SMD resistor that was on the back of the board?

Yes it was on the back of the board and so small would be easy to miss
