A simple error in assembly starts a chain of events that lead to a disaster.
NTSB released the report on the Dali, and here Jeff details how a misplaced number label prevented one wire from seating properly in a connecter.
The result was massive damage and loss of lives.

It happens sometimes, I tell all my guys to tug on the wire after they land it. They probably would have found it before it became a problem...

This was a classic "swiss cheese" failure. Multiple boo-boos had to all line up with each other just right. Turns out a redundant system would have kicked in and prevented loss of rudder authority and propulsion if it had been configured properly. That one bad connection should have had a backup and didn't.
